Chelsae R. Urrabazo, MLS, BSN, RN, is the founder and nurse compliance consultant for Country Life Assisted Living, a 16-bed Type B assisted living facility serving Medina, Atascosa, Bexar, and surrounding counties. A licensed registered nurse in Texas with more than 25 years of experience in geriatric healthcare and long-term care, she brings a unique combination of clinical expertise, healthcare operations experience, regulatory knowledge, and legal education.

As founder of Country Life Assisted Living, Chelsae developed the organization’s vision, policies, and operational framework, helping establish a resident-centered care model focused on quality, safety, and regulatory compliance. In her current consulting role, she provides strategic guidance on healthcare compliance, risk management, policy development, quality improvement, and regulatory readiness, while supporting the facility’s leadership team.

Chelsae began her nursing career in 2001 with Baptist Health System in geriatric home health and has held leadership and administrative roles across the continuum of care. She earned her Bachelor of Science in Nursing from Galen College of Nursing and her Master of Legal Studies in Healthcare Law, Policy & Management from Texas A&M University School of Law. Her legal education enhances her expertise in healthcare compliance, regulatory analysis, risk mitigation, and healthcare operations.

She has also completed specialized training through the Texas A&M Center of Excellence in Forensic Nursing, meeting the educational requirements for certification as a Sexual Assault Nurse Examiner – Adult/Adolescent (SANE-A), further strengthening her knowledge of healthcare investigations, documentation, and regulatory processes.

Chelsae remains actively engaged in healthcare leadership, quality improvement, and regulatory compliance initiatives, allowing her to bring contemporary industry perspectives and best practices to Country Life Assisted Living.

Chelsae is currently pursuing a Doctor of Nursing Practice (DNP) in Leadership at Pennsylvania State University, with a focus on systems leadership, quality improvement, and healthcare innovation.

A lifelong resident of Medina County, Chelsae lives on her family ranch with her husband and children. Outside of her professional work, she enjoys ranch life, cooking, traveling with her family, and hosting gatherings.

Business Consultant & Support Team Member

Joey Urrabazo joined the Country Life Assisted Living team in 2015 after a successful career in teaching and coaching. Although he has since transitioned to a full-time analyst role in the energy sector, he remains an integral part of the Country Life team, providing business, operational, and strategic support as needed.

Joey graduated from Trinity University with a Bachelor of Science in Business Management, where he also played college football. He later earned his Master of Business Administration (MBA) from Western Governors University. Continuing to expand his professional expertise, Joey is currently pursuing a Master of Supply Chain Management through Penn State University.

Born and raised in Medina County, Joey now raises his family on the family ranch. He is married to Chelsae R. Urrabazo, and together they have three children. Outside of work, Joey enjoys ranch life, cooking, sports, hunting, and planning family vacations.


Ricardo A. Montemayor, MDRicardo A. Montemayor, MD

Facility Medical Director

Dr. Montemayor serves as the medical director at Country Life Assisted Living. He has been a licensed physician in Texas since 1991 and operates a private practice in San Antonio, known as Montemayor Internal Medicine. 

Dr. Montemayor is a board-certified internal medicine physician who has served the San Antonio community for over thirty years. He is dedicated to treating his patients with compassion, dignity, and honesty and offers a wide range of clinical services to patients from adolescents to geriatrics. Dr. Montemayor is fluent in both English and Spanish.

Dr. Montemayor graduated in 1990 from the University of Texas Health Science Center at Houston Medical School.

Dr. Montemayor was born and raised in Bexar County, where he now raises his family. When he’s not working, he enjoys spending time with his loved ones, traveling, cooking, playing sports, and volunteering as a ringside physician at local amateur and professional boxing matches.

Christy joined the Country Life Assisted Living management team in 2015. She has collaborated with Chelsae, the owner, for more than 15 years in various healthcare areas. 

Christy has 15 years of experience in various aspects of geriatric healthcare. She started her career at Atascosa Health Center in 2010.

Christy was born and raised in Atascosa County, where she currently raises her family. She and her husband, Efraim Gutierrez, have five children. When she’s not working, Christy enjoys spending time with her family, attending church, cooking, practicing yoga, and enjoying moments with her grandchildren.
