Chelsae Renae Urrabazo, BSN, RN

Administrator/Registered Nurse

Chelsae is the sole owner of Country Life Assisted Living. Her roles include administrator and nurse. She is a licensed registered nurse in the state of Texas.

Chelsae has 20 years of experience in multiple areas of geriatric healthcare. She began her career with Baptist Hospital in geriatric home health in 2003. Chelsae is a Galen College of Nursing graduate with a bachelor of science in nursing.

Chelsae was born and raised in Medina County and now raises her family there. She is married to Joey Urrabazo; they have three children together. When not working, she enjoys spending time with her husband and children on the family ranch, cooking, sports, planning vacations, and holiday gatherings.

Joey joined the Country Life team in 2015 when he retired from teaching and coaching.

Joey is a Trinity University graduate who earned his bachelor of science in business management and played college football.

Joey was born and raised in Medina County and now raises his family there. He is married to Chelsae Urrabazo; they have three children together. When he is not working, he enjoys spending time on the family ranch with his wife and children, cooking, sports, hunting, and going on vacations.

Ricardo A. Montemayor, MD

Facility Medical Director
Dr. Montemayor is the facility medical director at Country Life Assisted Living. He is a licensed physician in the state of Texas since 1991.

Dr. Montemayor is a board-certified internal medicine physician who has been serving the San Antonio community for more than 20 years. Committed to treating his patients with compassion, dignity and honesty, he provides a broad scope of clinical services to patients ranging in age from adolescents to geriatrics. Dr. Montemayor is fluent in English and Spanish.

Dr. Montemayor graduated in 1990 from The Medical School of the University of Texas Health Science Center in Houston.

Dr. Montemayor was born and raised in Bexar County and now raises his family there. When he is not working, he enjoys spending time with his family, traveling, cooking, sports, and volunteering his time as a ringside physician in local amateur and professional boxing matches.

Christy Ann Gutierrez

Christy joined the Country Life Assisted Living management team in 2015. Christy has worked with Chelsae, the owner, for over 12 years in other areas of healthcare.

Christy has 12 years of experience in multiple areas of geriatric healthcare. She began her career with Atascosa Health Center in 2012.

Christy was born and raised in Atascosa County and now raises her family there. She and her husband, Efriam Gutierrez, have five children. When she isn’t working, she enjoys spending time with her family, church, cooking, yoga, and being with her grandchildren.
